The internal system is always parallel to project north as far as I'm aware. So when people use rotate project north command they are actually rotating all the elements within the model about the internal system, hence why as you say it often gets wrong sections, isolated foundations and text notes etc.

Rotate Project North affects plan views whose Orientation property is defined as Project North. It does not affect drafting views, callouts of plan views, or other types of views. The following example shows the model before and after using the Rotate Project North tool. Because it's inconvenient to have to work in a model that is slightly rotated, Revit defaults to a convenient north that aligns with vertical in the drawings.
